Personal Licence Forms

A Personal Licence authorises the holder to supply alcohol or authorises the supply of alcohol in accordance with a premises licence. A Personal Licence is a requirement for any person wishing to become the Designated Premises Supervisor (DPS) of a premises licensed to sell alcohol.

Application requirements

  • Applicants must be aged 18 years or over.
  • Must posses and produce an original Bii Level II examination certificate or a similar accredited qualification such as the EDI NCPLH level 2 qualification.
  • Must produce a Basic Criminal Conviction Disclosure, this must not be more than one month old when submitted with the application. This can be obtained by ringing 0845 6446676 or from www.disclosurescotland.co.uk
  • Supply 2 passport sized photographs with the application. One of these must be endorsed by a person in authority stating "This is a true likeness of (Applicants name)", signed, dated and state position of authority held. Applicants are also requested to complete a photograph certification form to ensure the photograph provided has been correctly endorsed. To make an application please complete both the application form and the disclosure form and send them together with the Criminal Conviction Disclosure (from Disclosure Scotland), Bii Level II certificate (or similar qualification), photographs and the fee of £37 (cheque or postal Order made payable to Basingstoke and Deane Borough Council). You may also pay over the telephone or on the councils website at www.basingstoke.gov.uk using the online payments facility.

Convictions

If during the application process or after a personal licence has been granted you are convicted of a 'relevant' offence, you must notify the court at the time you appear that you hold a personal licence issued under the Licensing Act 2003. You must also notify the Licensing Authority that issued your personal licence. Failure to do either can result on conviction in a fine not exceeding £500.
